- MBBS certificate
- PG/DNB certificate
- Registration with Karnataka State Medical Council
- Caste certificate of one-year senior residency for assistant professor post
- Proof of ID (Aadhar card and pan card)
- Passport size 2 photograph
- other documents/publications as per criteria for faculty appointment.
